Google Analytics provides statistical data about website visitors. I use the program to view the number of visitors to my website.
To schedule an appointment, I ask for your name, email address, and phone number. I use this information to contact you if the scheduled appointment needs to be changed or to inform you about matters related to our appointment. Additionally, I may use your contact details after the treatment to follow up for feedback, aftercare, or to arrange a follow-up appointment. Your phone number and email address are stored in an Excel file. Only I have access to this client database. Your contact details are also used for financial administration purposes.
Client File: For proper treatment, it is necessary that I, as your practitioner, keep a client file. This file contains notes about your health condition, both physical and mental. I do my best to safeguard your privacy. This means, among other things, that I handle your personal and medical information with care and ensure that unauthorized persons do not have access to your data.
As your practitioner, I am the only one with access to the information in your file. I am bound by a legal duty of confidentiality (professional secrecy). The information in your file may also be used for the following purposes:
– to inform other healthcare providers, for example when referring you to another practitioner. This only happens with your explicit consent.
– for a substitute colleague during my absence, if necessary.
– for anonymized use during peer review sessions.
If I wish to use your information for any other reason, I will inform you first and ask for your explicit consent.
You have the right to view your file and/or request its deletion. You also have the right to have your contact details removed from my records. I kindly ask you to notify me of this by email. By booking a treatment, you acknowledge that you have read and understood the above privacy statement.
